Tim Hortons credit card migration

Hi,

I saw a notice within the Tim Hortons app that the Tim Hortons credit card is being discontinued and will be migrated to Neo. At the time I read the message that I was not home, so I dismissed the prompt and hoped to read the details later.

After when I tried to go back in the Tim Hortons app again, that prompt no longer showed up. The link on the app now does not contain any information about the migration and only offers general information about the card.

Can you repost the link to the information and submission forms regarding the migration process?

Aside: this is what I saw within the Tim Hortons app but once I dismissed it, it does not re-appear again
